Approvers must open the plan, and the inbox becomes a notification centre

Approve and Reject are only accepted once the approver has opened the plan; the bell, the sidebar count and the Needs Action tab now agree.

## Open before you approve
An approver has to open the plan before Approve or Reject is accepted — in both approval rounds.

## One inbox, one count
The bell, the sidebar badge and the Needs Action tab now share a single definition of what needs your attention.

## Fixes and improvements
- The equipment audit trail records what changed, from what to what.
- The status field is gone from the equipment edit dialog — status is computed from active plans, not typed.
- A small number of bug fixes.
