Every isolation point gets its own QR code

Scan the point, not just the machine — with points-only plans, a scan-every-point template mode and two-person verification from the camera landing.

## A QR sticker per isolation point
Each isolation point has its own QR code. Scanning the parent equipment no longer verifies a point — the point itself has to be scanned.

## Points-only plans
A template can populate a plan with isolation point rows only, without an equipment header row.

## Scan each isolation point
A new template mode requires both the machine and every one of its points to be scanned independently.

## Verify from the camera landing
Scanning a point sticker with a phone lands on an actionable page, including two-person countersigning per point.

## Fixes and improvements
- A small number of bug fixes.
