Execution on the phone, refined

The isolation points table and the mobile execution screens were reworked so long text wraps, headers stay put and the next point is one tap away.

## Fixes and improvements
- Entering digital execution mode takes one tap, not two.
- A Next Point button replaces auto-advance on the phone, and the sticky header stays out of the way.
- Smart fields can be chosen as the tile title on the phone.
- A small number of bug fixes.
