Lockout instructions on equipment, with a proper editor

Write lockout instructions against a piece of equipment in a rich-text editor, and carry them into every plan that uses it.

## Instructions that live with the equipment
Each piece of equipment can carry its own lockout instructions — headings, lists, images and tables — written in a rich-text editor and shown in every plan built for that equipment.

## Fixes and improvements
- The equipment dropdown in the create-plan dialog is easier to use.
- A small number of bug fixes.
