Lockout on the phone, redesigned

The lockout screens were redesigned for phones, and equipment can carry its own per-machine isolation overrides.

## Lockout screens built for the phone
The lockout plan and execution screens were redesigned for phones: bigger targets, tiles instead of tables, and the actions you need at the bottom of the screen.

## Per-machine isolation overrides
A piece of equipment can override its isolation instructions individually, so a plan follows the machine in front of you.
