How to Write a JSA: A Step-by-Step Guide for Safety Managers

A Job Safety Analysis (JSA), also called a Job Hazard Analysis (JHA), is the most widely used tool for identifying hazards at the task level before anyone gets hurt. Here's how to write one that actually works on the shop floor, and holds up in front of an OSHA inspector.

Ask ten safety managers whether OSHA requires a JSA and you'll get ten different answers. Here's the honest one: no OSHA standard requires a document called a "JSA" or "JHA" by name. There is no line in 29 CFR that prescribes the form.

But that's not the end of the story, because OSHA absolutely requires the work a JSA does:

  • The General Duty Clause (Section 5(a)(1) of the OSH Act) requires every employer to keep the workplace free from recognized hazards likely to cause death or serious harm. When inspectors investigate an incident, the absence of documented hazard analysis is routinely used as evidence that hazards weren't identified or controlled.
  • 29 CFR 1910.132(d) requires employers to assess the workplace for hazards that call for PPE, and to certify that assessment in writing, naming who performed it, the workplace evaluated, and the date.
  • 29 CFR 1926.21(b)(2) requires construction employers to instruct workers in recognizing and avoiding the hazards of their work. A completed, signed JSA is the accepted evidence for all three. That's why a JSA program isn't paperwork theater. It's the cheapest legal protection and the most practical hazard-control tool a site can run. OSHA's own methodology, published as OSHA 3071 (Job Hazard Analysis), is the playbook inspectors and consultants reference, and it's what the steps below are built on.

Step 1: Pick the right jobs to analyze

Not every task needs a JSA, and trying to write one for everything is the fastest way to kill the program. Prioritize:

  1. Jobs with a history. Anything that has produced injuries, near misses, or first-aid cases.
  2. Jobs where one mistake is catastrophic. Tasks where a single error could cause serious injury or death, even if nothing has ever happened. Think energized equipment, work at height, confined spaces.
  3. New or changed jobs. New equipment, new process, new chemical, new crew. Hazards live in change.
  4. Complex or non-routine work. Tasks complicated enough that written instructions are needed, or performed rarely enough that nobody remembers the hazards. Start with your OSHA 300 log and near-miss reports. They'll tell you exactly where to begin.

Step 2: Involve the people who do the job

This is the step most JSAs skip, and it's why most JSAs fail. A hazard analysis written alone at a desk describes the job as the procedure imagines it, not as it's actually performed at 2 p.m. on a Friday with a production deadline.

Bring in at least one worker who performs the task. They know the shortcuts, the awkward reach, the guard that gets removed because it slows things down. Involving workers does two things at once: it surfaces the real hazards, and it builds ownership. A crew that helped write the JSA follows it. A crew that had it handed to them files it.

Before you observe the job, review its accident and injury history together. Past incidents are hazard data you've already paid for.

Step 3: Break the job into steps

Watch the job being performed. Don't reconstruct it from memory. Record the task as a sequence of steps, and follow two rules:

Describe what is done, not how. "Position ladder against tank" is a step. Three paragraphs on ladder angle is a procedure, and that belongs elsewhere.

Aim for roughly 10 to 15 steps. Fewer than that and you're compressing multiple actions into one line, which hides hazards. Many more and the document becomes a novel nobody reads. If a job genuinely needs 25 steps, it's probably two jobs. Split it into two JSAs.

When you're done, walk the steps back through with the worker you observed. Missing a step means missing every hazard in it.

Step 4: Identify the hazards in each step

Now go step by step and ask one question: what could go wrong here?

Work through the common hazard categories as a prompt list:

  • Struck-by (falling objects, moving equipment, flying particles)
  • Caught-in or caught-between (pinch points, rotating parts, collapse)
  • Falls (from height, same-level slips and trips)
  • Electrical contact
  • Chemical exposure (inhalation, skin contact, splash)
  • Hot and cold surfaces or environments
  • Stored energy (pressure, tension, gravity, electrical, anything a lockout would control)
  • Ergonomic strain (lifting, awkward posture, repetition)
  • Noise and vibration The discipline that separates a useful JSA from a vague one: name the consequence, not just the hazard. "Rotating shaft" is a noun. "Contact with rotating shaft: entanglement, laceration or amputation" tells the reader why the control matters. OSHA's guidance describes good hazard statements as capturing the environment, who's exposed, what triggers the event, and what the consequence is. You don't need that formalism in every cell, but every hazard line should answer what happens to whom.

Step 5: Choose controls using the hierarchy, then keep the document alive

For each hazard, decide how to eliminate it or reduce it to an acceptable level. Work down the hierarchy of controls, in order:

  1. Elimination. Remove the hazard entirely. Can the task be done at ground level instead of at height?
  2. Substitution. Swap the hazard for something less dangerous. A less volatile solvent, a lower-pressure system.
  3. Engineering controls. Guard it, ventilate it, isolate it. The hazard still exists, but workers can't reach it.
  4. Administrative controls. Procedures, permits, training, rotation, signage.
  5. PPE. The last line of defense, never the first answer. Most weak JSAs jump straight to "wear gloves and safety glasses" for every step. If your controls column is nothing but PPE, the analysis hasn't been done. It's been decorated. Push each hazard as far up the hierarchy as is feasible, and only then specify PPE for the residual risk. As a side benefit, a JSA whose hazards map cleanly to specific PPE requirements satisfies the written hazard assessment certification that 29 CFR 1910.132(d)(2) demands. One document, two obligations covered.

Then close the loop:

  • Review the finished JSA with the whole crew and have everyone sign an acknowledgment. Under 1926.21(b)(2), that signature page is your evidence that hazard instruction happened.
  • Treat it as a living document. Re-review the JSA after any incident or near miss on the task, whenever equipment, materials, or methods change, and at least annually even if nothing changed. A JSA dated three years ago describes a job that no longer exists.

Common mistakes to avoid

Writing it once and filing it. An out-of-date JSA is arguably worse than none. It documents that you knew about the job and stopped paying attention.

Copy-pasting between tasks. Inspectors spot boilerplate instantly. If two different jobs have identical hazards and controls, one of the analyses wasn't performed.

Steps that are really procedures. The JSA identifies hazards and controls; it isn't the work instruction. Keep the two linked but separate.

No worker signatures. Without the acknowledgment section, you can't demonstrate that the analysis ever reached the people it was written to protect.

Paper JSAs that never come back. The failure mode of paper isn't writing the JSA. It's version control, review dates that silently pass, and signed copies that live in a binder nobody opens until after the incident.
