No. Observations are advisory by design. An open observation never stops a job from closing or a permit from issuing. It gives the safety team visibility and a tracked follow-up loop. Whether work should stop stays a human call.
A triage owner assigns each action to a person with a due date. The assignee marks it done, with a note or photo of the fix. The safety team then verifies it. The report can only close once every action is verified, so nothing is quietly waved through.
